Intracytoplasmic Morphologically Selected Sperm Injection (IMSI)

What is IMSI?
Sperm morphology is very important for the proper diagnosis of male fertility potential. IMSI literary means the intracytoplasmic injection of morphologically selected sperm cells. It is combined with MSOME - Motile Sperm Organelle Morphology Examination. Up to now, this is the only technique that allows the real-time selection of the best sperm cells according to their motility, morphology and organel characteristics, for intracytoplasmic injection (ICSI).

This method was developed in 2001. It requires the use of an inverted microscope with optic contrast system, digital camera, and more than 6000x magnification. This is much higher than the standard 200x or 400x magnification used in ICSI procedures. This is a non-invasive approach for selection of the morphological best sperm cells in order to support the fertilization process. This also improves the embryo development in couples with severe male infertility factor as the main reason for previous ICSI failures.

How does IMSI help?
The selection of sperm cells with normal morphology for ICSI is connected to the higher quality of the blastocysts, higher implantation, and pregnancy rates, and decreases the miscarriage rate.

When is advisable?
IMSI is usually applied in cases of:
- Previous ICSI failures
- Severe male infertility factor (severe Oligoasthenozoospermia or morphology lower than 4% according to the Kruger’s criteria)
- High levels of DNA fragmentation
- A small number of embryos with good quality on day 5 in previous IVF/ICSI procedure
